The Challenge

They had an MVP built by a freelancer. It worked — barely. Scheduling was broken half the time, video calls dropped on mobile, and HIPAA compliance was mostly theoretical. They needed to go from 'we have something' to 'we can show this to hospital partners' in under 4 months.

What We Did

Full rebuild over 14 weeks. We replaced the video layer with a properly integrated Twilio implementation (the original used a self-hosted WebRTC setup that couldn't handle NAT traversal reliably). Rearchitected the scheduling system to handle timezone edge cases. Ran a proper HIPAA compliance audit with a third-party security firm — we coordinated, they paid. The mobile app was rebuilt in React Native with offline appointment caching.

The Result

50K active users within 8 months of relaunch. 10K monthly consultations with a 94% completion rate. App Store rating went from 3.1 to 4.8. First hospital partnership signed 6 weeks post-launch.

What We'd Do Differently

Migration of existing user data took 3 weeks longer than planned. We underestimated the inconsistency in the legacy database schema — appointment times were stored in at least 4 different formats. Should have run a data audit earlier in the project.
